P.149 High-fidelity simulation-based microsurgical training for neurosurgical residents

Background: Mastery of microsurgical skills requires regular hands-on practice. Simulation is increasingly recognized as an important supplement to operative training experience. The live rat femoral artery model the gold standard for microneurosurgical simulation. We present 11-year experience incorporating simulation-based into established Canadian neurosurgery program. Methods: Postgraduate year 2 (PGY-2) residents completed a one-year curriculum spanning 17 sessions divided 5 modules increasing fidelity. Both perfused duck wing and vessel were used. Three comprised microvascular anastomosis. Trainee performance was video recorded blindly graded using Objective Structured Assessment Technical Skills Global Rating Scale. Results: Eighteen participants 107 anastomoses. There significant improvement in six measurable during curriculum. Mean overall score significantly higher on fifth attempt compared first all 3 anastomotic (p<0.001). Each module had different profile across assessed. greatest observed artery-to-artery Conclusions: This high-fidelity simulation demonstrated assessed, supporting its use effective teaching model. Transferability environment actively being investigated.
